Coverage and Payment

• At no obligation or cost, BioTAB Healthcare will verify your coverage & benefits to determine if we can provide the Gradient Sequential Lymphedema Pump.

• In almost every case we are able to accept the final amount your plan pays after all deductibles and co-pays. As a result, we often can provide the pump with NO co-pay deposit or out of pocket expense to our patients

• We will coordinate a letter of medical necessity or prescription from your physician, obtain a pre-authorization with your insurance plan when necessary, and submit the paperwork.

• Our aim is to provide only the best possible REIMBURSED products for our patients.

• The medical determination process is complicated and different for Medicare and Private insurance carriers, however it is a covered benefit under all insurance policies.
